The difference is one amino acid. Here is what that does mean, what it does not, and why it matters less for ghee than for milk.
Beta-casein is one of the main proteins in cow milk, a chain of about 209 amino acids. At position 67 of that chain, some cows produce histidine and some produce proline. The histidine version is called A1; the proline version is called A2. That is the whole difference — one position, one amino acid.
It is not nothing. That substitution changes where the protein splits during digestion, and the A1 form can release a peptide called beta-casomorphin-7 (BCM-7) that the A2 form does not release in the same way. BCM-7 is real, measurable, and the subject of most of the argument.
Indian desi breeds — Gir, Sahiwal, Red Sindhi, Tharparkar, Kankrej — are predominantly A2. So are buffalo, goats and sheep. The A1 variant arose as a mutation in European cattle and is common in high-yield breeds such as Holstein Friesian, and therefore in the crossbred herds that produce most of India's commercial milk.
A cow's variant is genetic. It cannot be changed by feed, by grazing, or by how kindly the animal is kept — all of which matter enormously for milk quality, and none of which are what "A2" refers to.
Here is the fact that ought to appear on every A2 ghee page in India and almost never does.
Ghee is clarified butterfat. Making it means cooking off the water and straining out the milk solids — and casein is a milk solid. A properly made ghee is roughly 99 to 99.5 per cent milk fat. Whatever is true about A1 versus A2 beta-casein in a glass of milk applies to a spoonful of ghee only in traces.

Because breed is a proxy for a whole production system. Desi-breed milk in India generally comes from smaller herds, from animals that are not pushed for volume, often on mixed feed and open grazing. None of that is guaranteed by the letters "A2" — but the herds are different herds, and the milk is handled differently.
Treat A2 as a reliable claim about provenance and a weak claim about chemistry. Then ask the questions that actually separate one ghee from another: which breed, whose herd, curd churned or cream boiled, which laboratory, which batch.

This is a different question and worth separating. Lactose is a sugar, not a protein — the A1/A2 distinction has nothing to do with it. Ghee is very low in lactose because the milk solids are removed, which is why many people who react to milk find ghee sits fine. That is a statement about how ghee is made, not about the breed of cow. If you have a diagnosed dairy allergy, which is a protein response rather than a sugar one, ask a doctor rather than a ghee company.

Not meaningless, but it means something different from what it is usually sold as. A2 tells you the milk came from a desi breed, and desi-breed milk is typically from smaller herds, different feed and different husbandry. It is a reliable signal about provenance. It is a weak one about the chemistry of the finished ghee.
Studies have reported differences in digestive comfort between A1 and A2 milk in some people, and beta-casomorphin-7 — a peptide released when A1 beta-casein is digested — is a real and measured compound. Beyond that, findings on wider health outcomes are mixed, several trials are small or industry-funded, and food authorities including FSSAI have not endorsed health claims for A2 milk. Anyone telling you it is settled is overselling.
No, and they get conflated constantly. A2 is about the breed of cow. Bilona is about the method — curd churned to butter, butter simmered to ghee, rather than cream boiled directly. A ghee can be one, both or neither. Ask separately.
